Bugs & Other Improvements

Continuity of Care Document (CCD) (.xml file)

Meaningful Use Stage 2 Object 7 - Medication Reconciliation

  • We've resolved an issue that caused the denominator of Stage 2 Objective 7 in the Meaningful Use report to inadvertently inflate. The system was adding a new "Transition of Care" document to the patient's chart each time a Continuity of Care Document (CCD) (.xml file) was assigned and reconciled for a patient. The denominator was including these multiple documents in its calculation. Now, the calculation is working as expected. The assignment and reconciliation of patient documents from the EHR Message Center adds only 1 "Transition of Care" document  to the patient's chart and increases the denominator by 1. To learn more about Transition of Care and CCD assignments checkout the following articles. Objective 7: Medication Reconciliation?    How do I receive and reconcile a Continuity of Care Document (CCD) from another provider?

  • We also corrected a bug that impacted the numerator which only increased when a new medication was added to a patient's chart. The measure is now satisfied when the provider documents that they have performed a medication reconciliation. The numerator will increase by 1 when the Confirm all or Confirm Medications button is selected during the Continuity of Care Document (CCD) (.xml file) assignment, you do not have to add a medication to patient's chart in order to satisfy the numerator for this measure.

Errors During CCD Reconciliation

  • We resolved the issue that caused the following error to occur during CCD Reconciliation: "Validation failed. The value in field 'Desc' in type TempReconciliations is too large. The maximum length is 200."

  • We also fixed issues that caused erroneous errors during CCD Reconciliation. One of the errors pertained to unknown Allergy types. Now, the system will list those Allergy codes so you can submit a support ticket to have them added.

Invoice

A bug was fixed that caused an invoice with an Rx order attached to automatically post if the Rx Order was opened, modified and saved.

EHR

  • A Reload button will now display in the EHR if the left-hand slider is blank so you can manually refresh the slider to its normal state. This is a random occurrence and we are continuing to research the cause.

  • You are now able to perform appropriate actions (e.g., Delete, Add Attachment) to EHR document records regardless of the signed status. A provider may need to use the delete action if multiple Transition of Care documents where created impacting their Meaningful Use reporting.
