Working with Patient Questionnaires

You can pre-populate the PFSH, ROS and Lifestyle sections of the exam form if a questionnaire was filled out by a patient or from a past Exam.

If a pre-filled form is available, a note will appear, listing which questionnaire was filled out along with the date.

You can not EDIT THE TEXT or the LIST on a questionnaire. Any custom questions added in the Admin section are not transferred over to the EHR.

1. Click Lifestyle, Review of Systems or Past Family and Social History buttons to open the questionnaire

2. A message will display at the top of the questionnaire if there is a questionnaire already on file

3. Select which questionnaire you would like to use to populate the answers

Questionnaire: Populate answers from the patient questionnaire

Last Exam: Populate answers from the most previous exam

Questionnaire & Last Exam: Populate answers from both. Notes from both the questionnaire and last exam will pull forward as well. If the questionnaire and last exam have conflicting answers, meaning the last questionnaire had one answer and the last exam had another, Uprise will flag those and default to the abnormal (default to yes)

4. If Questionnaire & Last Exam is selected and there are discrepancies Uprise will flag those

The conflicts aren't an error that need to be fixed before selecting Reviewed, but there to warn the user that there are conflicting answers.

5. Hover over the warning sign to see where the answers came from and then change the answer if needed

6. After selecting a questionnaire to populate the answers, edit the questionnaire as needed then click Reviewed

7. Under the Reviewer section a "Last reviewed by..." note will be updated
