How do I receive and reconcile a Continuity of Care Document (CCD) from another provider?

The Continuity of Care Document (CCD) is an electronic document exchange for sharing patient summary information between providers. A CCD can be received in Uprise and then matched to the patient record in your system. The CCD summary information can then be uploaded to your patient. Summary information includes patient Problems, Medications and Allergies.

1. Incoming CCD's are delivered to the Provider's Message Center In the EHR, Click Message Center > Click Inbox.

2. Double-click or tap the message.

3. The message opens. Confirm that the patient is listed in Uprise with a matching name, gender, and birth date.

NOTE: The name has to be exact, including "Sr.", "Jr.", etc.

4. There may be two or more attachments. The CCD will be a ".xml"

Note: There may be multiple ".xml" files but only one is the CCD. The others may be a Cover Letter or another type of document.

You may need to click Show CCD ".xml" to confirm which document is the CCD.

5. Double-click or tap on the patient name on the right hand side

6. Click Assign CCD and open Reconciliation and click Yes, indicating you're sure.

If you click Assign CCD and Close, the CCD is simply added to the patient record without consuming the CCD.

7. Confirm the CCD's information is coming through properly.

In the Reconciliation window you will find three tabs: Problems, Medications, Allergies. They each work the same way. On the left hand side you'll see the items that are coming in from the CCD and on the right hand side you'll see the items that are already in the exam history of this patient.

You can also review the CCD itself by clicking on CCD Source.

Confirm the CCD's information is coming through properly.

8. Reconcile the problems according to the options below.

8.1. To move an individual problem to the reconciled list, click the arrow to the left of it.

Note: It is not necessary to move all Problems. For example, if you already have the problem list on the patient record.

8.2. To move all problems at once, click Confirm Problems Reconciliation above the tabs.

9. Repeat steps 8.1 and 8.2 for the Medications and the Allergies.

10. Click Confirm All and confirm that you are sure.

11. The medication reconciliation requires medications be reconciled in e-Prescribing.

The e-Prescribing window will open automatically. To complete this portion, follow the steps below:

11.1. Review that the medications listed under Imported Rx and the allergies listed under Allergy / Intolerance match the import

11.2. Click the drug name > select the correct dosage
