How do I make changes to the Questionnaire?

You can remove a question or add a Custom question. You can not change the wording on questions.  All changes take place under the Admin tab

1. Admin > Setup > Intake Questionnaire > click the section you want to update

2. By default, all sections are checked. Uncheck section you do not want to display.

3. Add a custom question by scrolling to the bottom of the screen and clicking "Add Question"

4. Choose the type of question and then enter the desired responses
