How to Upload Forms to your Patient Portal
Practice forms can be uploaded to the Patient Portal and can also be signed electronically by the Patient.
1. Click Admin > Setup > Organizations > Upload
2. Locate and select the form you want to upload
3. Once uploaded you can set the Type of form, the end date, and the locations that will see the form.
If you upload a PDF, the form can be electronically signed and recorded in Uprise automatically.
