How to Create a New Patient

In this article, required and other important fields are identified. Meaningful use fields are also highlighted: Ethnicity, Race, Preferred Language, Preferred Communication Method and Recall Communication Method.

1. Click Patients Tab > Create New Patient Button

2. Begin entering information

3. Enter required Demographics

5. Enter Communication Method(s)

Note: whatever communication method you enter in first is considered the patient's primary contact in Uprise.

5.1. Click Add button to enter an Email address.

An email is needed to create a Patient Portal account

Click Add button to enter an Email address.

5.2. Enter Preferred Recall Communication Method > Reason

5.3. Enter Preferred Recall Communication Method > Delivery Method and Type

6. When information is complete, click Save (upper right corner)
