Promoting Interoperability

The following Promoting Interoperability (PI) qualifiers are automatically calculated in Uprise based on the following criteria. Uprise supports two measure sets for submitting data: PI Measures and PI Transition Measures. For the 2018 Performance Period attestation eligible clinicians must use the PI Transition Measure Set, for the 2019 Performance Period eligible clinicians must use the PI Measure Set.
PI Measures
1. e-Prescribing
At least one permissible prescription written by the MIPS eligible clinician is queried for a drug formulary and transmitted electronically.
In Uprise: This is achieved through adding a medication through the orders section and selecting Rx Medication order type. Composing a Medication Prescription
2. Provide Patient Access
For at least one unique patient seen by the MIPS eligible clinician: (1) The patient (or the patient authorized representative) is provided timely access to view online, download, and transmit his or her health information; and (2) The MIPS eligible clinician ensures the patient's health information is available for the patient. Setup Patient Portal.
In Uprise: Patient must login to Patient Portal and enter the Security Token
3. Health and Information Exchange
For at least one transition of care or referral, the MIPS eligible clinician that transitions or refers their patient to another setting of care or health care provider-(1) creates a summary of care record using certified EHR technology; and (2) electronically exchanges the summary of care record.
In Uprise:
Create and send a referral order with a CCD attached. There are two workflows that will count/trigger score 1) before exam signoff electronically sending CCD through Examination order window, 2) after exam signoff, electronically sending CCD.
When attesting this report is "Support Electronic Referral Loops by Sending Health Information."
4. Receive and Reconcile
For at least one transition of care or referral received or patient encounter in which the MIPS eligible clinician has never before encountered the patient, the MIPS eligible clinician receives or retrieves and incorporates into the patient's record an electronic summary of care document.
In Uprise: Reconcile an incoming CCD with generate a Transition of Care (TOC) document in the patients Documents tab. Note - When reconcile a CCD a TOC document is automatically inserted into the Documents tab.
When attesting this report is the "Health Information Exchange - Support Electronic Referral Loops by Receiving and Incorporating Health Information".

5. Health Information Exchange
For at least one transition of care or referral, the MIPS eligible clinician -(1) creates a summary of care record using certified EHR technology; and (2) electronically exchanges the summary of care record.

6. Receive And Reconcile
For at least one electronic summary of care record received for patient encounters during the performance period for which a MIPS eligible clinician was the receiving party of a transition of care or referral, or for patient encounters during the performance period in which the MIPS eligible clinician has never before encountered the patient, the MIPS eligible clinician conducts clinical information reconciliation for medication, mediation allergy, and current problem list.
In Uprise:
Receive an incoming CCD and perform reconciliation for medications, medication allergies, and current problems. Note - When reconcile a CCD a TOC document is automatically inserted into the Documents tab.
General note:
- Required Attestation measures: Prevention of Information Blocking, ONC Direct Review, completed the Security Risk Analysis
- Required objective ‘Public Health and Clinical Data Exchange’ includes: Immunization registry reporting, syndromic survelliance reporting, electronic case reporting, public health registry reporting, clinical data registry reporting.
- Failure to submit a numerator of at least one for measures or claim an exclusion will result in a zero being earned for the Promoting Interoperability performance category.
- When attesting this report is the "Health Information Exchange - Support Electronic Referral Loops by Sending Health Information".
7. VDT - View, Download, Transmit
At least one patient seen by the MIPS eligible clinician during the performance period (or patient-authorized representative) views, downloads or transmits their health information to a third party during the performance period.
In Uprise: First the provider has to compose the CCD after exam signing, which will send the CCD to patients with an active portal account. Then the patient will need to download, print, or transmit the Visit Summary within Patient Portal. Having access to Uprise Application Access/Certification API will count for this report.
8. Patient-Specific Education
The MIPS eligible clinician must use clinically relevant information from certified EHR technology to identify patient-specific educational resources and provide electronic access to those materials to at least one unique patient seen by the MIPS eligible clinician.
In Uprise: If a patient has an active Patient Portal, when education materials are added to the Exam, they are sent to patient's portal once the Exam is signed.
9. Secure Messaging
For at least one unique patient seen by the MIPS eligible clinician during the performance period, a secure message was sent using the electronic messaging function of certified EHR technology to the patient (or the patient-authorized representative), or in response to a secure message sent by the patient (or the patient-authorized representative)
10. Medical Reconciliation
For at least one transition of care or referral received or patient encounter in which the MIPS eligible clinician has never before encountered the patient, the MIPS eligible clinician performs clinical information reconciliation. Review of the patient's medication, including the name, dosage, frequency, and route of each medication.
11. Patient-Generated Health Data
Patient-generated health data or data from a non-clinical setting is incorporated into the certified EHR technology for at least one unique patient seen by the MIPS eligible clinician during the performance period.
